3 quick steps

To work as a doctor in Germany with a foreign degree, you usually need to obtain Approbation, a permanent license to practice medicine. Before full Approbation, Berufserlaubnis may be possible in some cases as a temporary work permit with restrictions. If the degree is not recognized as fully equivalent to German medical education, the candidate may be assigned a Kenntnisprufung, an exam to confirm medical knowledge.

The path depends on the country where the degree was earned, the federal state, the chosen specialty, language, documents, and whether you also need a visa or residence permit. That is why it makes more sense to think not in terms of a “single procedure for Germany,” but of a route through the specific competent authority in the federal state you choose.

Two main routes

There are two different scenarios for a future medical practice:

  • you already have a medical degree and go through recognition in order to obtain Approbation;
  • you enroll in a German medical university and earn the education again, or possibly receive credit for individual stages if the university allows it.

The first path is usually chosen by doctors who have completed their education and already have experience. The second path suits those who are only planning a medical career or cannot use their existing degree to gain admission to practice.

Route 1: recognition of a foreign medical degree

Approbation gives the right to work as a doctor in Germany without a temporary time limit. To obtain it, the competent authority compares the foreign education with German medical training and checks additional conditions: personal reliability, health status, knowledge of German, and the right to engage in professional activity.

The package usually includes:

  • a medical degree and the supplement showing subjects, hours, and grades;
  • documents on practical training, internship, residency, or work experience, if available;
  • proof of professional experience;
  • a passport and, if necessary, residence documents;
  • a certificate of no criminal record or an equivalent document;
  • a medical certificate confirming fitness to work;
  • German language certificates;
  • certified translations and copies according to the requirements of the specific authority.

The exact list of documents must be taken from the authority in the federal state where the application is submitted. Some states require additional forms, while others view the validity periods of certificates, translations, or proof of intent to work specifically in that state differently.

What happens after the application is submitted

After the documents are received, the authority checks whether the education can be recognized as equivalent. The outcome may vary:

  • full equivalence and further processing of Approbation;
  • identified substantial differences that must be compensated for;
  • a request for additional documents;
  • assignment of a Kenntnisprufung if the differences cannot be resolved through documents and experience.

Professional experience may be taken into account, but it does not always replace missing parts of the education. That is why it is important not just to collect diplomas, but to show the content of the training and practice: hours, disciplines, clinical blocks, duties, and verified work experience.

Kenntnisprufung: when the exam is needed

The Kenntnisprufung is assigned when the authority believes there are substantial differences between the foreign training and German medical education. This is not a language exam, but a test of professional knowledge according to German standards. In practice, special attention is usually paid to clinical thinking, internal medicine, surgery, emergency situations, pharmacology, and the legal and organizational aspects of the German healthcare system.

It is worth preparing not only from textbooks. It is important to understand German documentation, the structure of a hospital, and interaction with patients, insurance providers, colleagues, and the on-call service. That is why many doctors use a period of temporary work or an internship for adaptation.

Berufserlaubnis: temporary authorization

Berufserlaubnis may allow a doctor to work before obtaining full Approbation, but it is not a complete replacement for Approbation. The permit is usually limited by time, place, employer, or type of activity. Work is often carried out under supervision and within the conditions specified by the authority.

This option is useful if you need to enter the German clinical environment, improve your professional German, and prepare for the next stage of recognition. But you should not rely on Berufserlaubnis as an automatic interim status: the decision depends on the federal state, the documents, the employer, and the specific situation.

Language requirements

For doctors, language is part of admission to the profession, not a formality. According to official information, the minimum general requirement in all federal states is German at no lower than B2 under the CEFR. In addition, a medical Fachsprachprufung at C1 level is usually required.

In practice, this means a doctor needs to be able to:

  • take a medical history and explain a diagnosis to a patient;
  • maintain medical documentation;
  • hand cases over to colleagues;
  • understand prescriptions, discharge papers, and protocols;
  • communicate in stressful and emergency situations.

General conversational German is often not enough. It is worth training medical vocabulary separately, as well as the structure of an Arztbrief, Aufklarungsgesprach, Ubergabe, and communication with patients of different ages.

Visa and work during recognition

Citizens of countries outside the EU, EEA, and Switzerland usually need a suitable residence status. Depending on the situation, options may include a work visa, an EU Blue Card, a visa for recognition of qualifications, or other statuses. For doctors, it is important that professional authorization and the immigration basis do not contradict each other: an employment contract alone does not give the right to treat patients without the necessary license.

If missing qualifications are identified during recognition, a route for completing qualification measures and a later exam may be suitable. It is better to check the specific option before signing a contract, especially if the employer expects a quick start in a clinical position.

Route 2: medical education in Germany

Getting a medical education at a German university is a longer path, but it leads to a German qualification. Medical studies usually last around six years and include a pre-clinical stage, a clinical stage, a practical year, and state exams. After graduation, the graduate applies for Approbation.

Admission to medicine is highly competitive. Factors include the school-leaving certificate, recognition of university entrance eligibility, German language skills, results of selection procedures, and the requirements of the specific university. For international applicants, Studienkolleg, TestAS, preparatory courses, application deadlines through uni-assist or directly to the university, and the Hochschulstart rules for certain categories may be important.

Exact passing scores should not be treated as fixed: they change by semester, university, and quota. So instead of relying on the idea that “only a perfect GPA is needed,” it is better to check in advance which criteria the university you choose actually uses.

How to improve your chances of admission

  • Check whether your school certificate is recognized for direct admission or whether you need Studienkolleg.
  • Prepare your German to a level sufficient for a medical program.
  • Clarify whether the university takes TestAS, practical experience, motivational elements, or additional tests into account.
  • Submit documents with time to spare: translations, certifications, and recognition of school documents take time.
  • Do not rely on a single university if the rules allow you to submit several applications.
  • Check current fees: public universities often charge a semester contribution, but some federal states and universities may charge additional fees for international students.

Common mistakes doctors make during preparation

  • Submitting an incomplete package of documents and losing months on additional submissions.
  • Underestimating the Fachsprachprufung and preparing only for general B2.
  • Choosing a federal state randomly without comparing the requirements and timelines of the authorities.
  • Assuming that experience will automatically replace all differences in education.
  • Signing a contract without understanding which license is needed for the specific position.
  • Postponing translations, certifications, and certificates until the moment of booking an appointment with the authority.

Practical order of steps

  1. Choose the federal state where you plan to work.
  2. Find the competent authority through the Recognition Finder or the list of medical licensing authorities.
  3. Download the current list of documents specifically for that federal state.
  4. Check which language certificates and Fachsprachprufung are accepted.
  5. Prepare your degree, supplement, work experience certificates, translations, and certifications.
  6. Apply for Approbation or clarify whether Berufserlaubnis is needed first.
  7. If a Kenntnisprufung is assigned, create a preparation and practical adaptation plan.
  8. Coordinate your immigration status with your professional authorization if you are not a citizen of the EU, EEA, or Switzerland.

FAQ

Can you work as a doctor without Approbation?

For full medical practice, Approbation is required. In some cases, Berufserlaubnis may be possible, but it is temporary and limited. The conditions are determined by the competent authority of the federal state.

Is B2 enough for a doctor?

B2 is usually the minimum general level of German. In addition, a medical Fachsprachprufung at C1 level is most often required. The specific rules and exam format depend on the federal state.

How long does recognition of the degree take?

There is no single time frame. The procedure depends on the completeness of the documents, the country of education, the workload of the authority, the need for an expert assessment, an exam, and the federal state of application. It is better to allow extra time and not tie your move to an overly optimistic scenario.

What should you choose: Approbation or studying again?

If you already have a full medical degree, the recognition route is usually checked first. Studying at a German university makes sense if you are only beginning your path in medicine, cannot confirm your qualifications, or consciously want to obtain a German education from scratch.
